• Am I reimbursed less if I use a doctor outside of the insurance company's network? If so, how much less?

Does the insurance plan cover the services I am likely to use?

• Are the doctors, hospitals, laboratories and other medical providers that I use in the insurance company’s network?
• Will the network permit me to use a doctor outside of the network?
• Is it easy to change my primary-care physician if I need to?
• Do I need to receive permission before seeing a medical specialist?
• What does the process involve for getting care and then being reimbursed in an emergency situation, when at home or even out of town?
• How does a preexisting medical condition affect my coverage?
• If I have a chronic condition such as asthma, cancer, AIDS or alcoholism, how will the plan treat it?
• Are the prescription medicines that I need covered by the plan?
• If I want to be treated through alternative medical therapies, will I be reimbursed?
• Does the plan cover the costs of giving birth?
